Who submits Referrals

• Members
• Case Managers from Clinics or Crisis Centers
• Licensed Professionals
• Hospitals or Other Behavioral Health Facilities
• Family, Friends, & Peers

What happens once Referral is received?

• Eligibility verification
• Individual is added to our system and assigned the closet location*
• The individual (by phone) and referring party (by email) will be notified when they are able to start attending services and if additional paperwork is needed
